In order to keep our patients and staff safe from exposure to COVID-19 we have implemented the steps below. This policy is based on standards and requirements set forth by the Center for Disease Control and Prevention, the Washington State Department of Health, and the federal Occupational Safety and Health Administration (OSHA) COVID-19 Emergency Temporary Standard (29 CFR 1910, subpart U).
All patients and visitors to Evergreen Women’s Health will be screened upon arrival via our COVID-19 Health Screening Form
Any YES answer to the following will be considered a sufficient reason to postpone their in-person visit:
- Have you had contact in the last 14 days with someone with confirmed COVID-19?
- Do you live in a facility that has COVID-19 confirmed cases in the last 14 days?
- Have you tested positive for COVID-19 in the last 14 days?
- In the last 14 days have you had any of the following that cannot be explained by an underlying medical condition: cough, shortness of breath, difficulty breathing, vomiting or diarrhea?
- In the last 14 days have you had any two or more of the following that cannot be explained by an underlying medical condition: fever, chills or rigors, muscle aches and pain, headache, sore throat, new loss of taste or smell?
Additionally, if a person has travelled in the past 10 days they may be asked to postpone their in-person visit. Travel is defined as any trip that is overnight AND on public transportation (plane, train, bus, OR any trip that is overnight AND with people who are not in your household). Travel incidents will be taken into consideration with other survey answers and the patient's vaccination status
If a patient’s visit is postponed due to the above reasons then the patient will be rescheduled for at least 14 days past the date of their positive screening.
